The body is doing precisely what it evolved to do in the face of threat. The problem is that it has misfired.
Why Panic Attacks Happen
The causes are layered and rarely reduce to a single explanation. Chronic stress is one of the most consistent contributing factors I observe clinically. When the nervous system has been running at high alert for weeks or months — through work pressure, relational conflict, grief, or health anxiety — the threshold for triggering that alarm lowers significantly. Genetics also play a role; if a parent or sibling experiences panic disorder, your own susceptibility is meaningfully higher. Certain physiological factors matter too: thyroid irregularities, stimulant use, sleep deprivation, and even excessive caffeine can all push a sensitised system over the edge.
Importantly, panic attacks do not always follow logic. They can arrive during calm moments — waking from sleep, sitting in a meeting, standing in a grocery queue. This unpredictability is part of what makes them so destabilising and why the fear of having another attack often becomes its own source of chronic anxiety.

When a panic attack is happening, the most effective immediate intervention is controlled breathing — specifically, extending the exhale beyond the inhale. Breathing in for four counts and out for six activates the parasympathetic nervous system and begins to interrupt the adrenaline cycle. Grounding techniques, such as naming five things you can see or pressing your feet firmly into the floor, help redirect the brain’s attention away from internal threat signals and back toward the present environment.
